Partnering for Value Based Care

Strategic partnerships are essential to delivering exceptional outcomes in the changing healthcare landscape. Our highly skilled clinical expertise complements care provided in acute settings, presenting a more economically effective setting for rehabilitation and post-acute care. Our skilled nursing affiliates develop and maintain specialized clinical competencies and protocols to meet the needs of the population served and the higher level of treatment now required in post-acute care settings.

Putting Strategy into Action

As a not-for-profit, we believe in reinvesting in our programs, people and services. Our strategic initiatives position our affiliates as a reliable, dependable partner for physician groups and ACOs. for successful value based care and true partnership. We have developed a 5-star strategy that focuses on continually improving quality to achieve successful value based care and the best possible outcomes for those we serve.

Our initiatives focus on reducing rehospitalization rates, short-term length of stay, and anti-psychotic use. We share best practices across affiliates and conduct internal process reviews and peer evaluations to identify areas for improvement. Capital improvements, expanded feedback from satisfaction surveys, and continued education in specialized clinical competencies support these efforts.

Taking the Lead with Collaborative Efforts

We collaborate with a variety of partners to explore new care delivery models and improve outcomes and accountability. Collaborative efforts between Integritus affiliates and the Massachusetts Senior Care Association include a transitions of care pilot program, coaching and DON training programs,and other quality initiatives. A palliative care program in partnership with Berkshire Medical Center’s Cancer Center helps patients undergoing treatment evaluate treatment options and make thoughtful decisions around goals of care.

We have piloted a Care Manager role at key affiliates to improve transition outcomes for short-stay populations. Proactive communication with our professional partners improves outcomes for patients transitioning home and delivers a higher level of accountability and customer service.

Other efforts include innovative ways to treat acute cardiac and CHF patients in a sub-acute setting, capabilities and policies around substance disorder treatment, and wound care initiatives.

Creating a Unique Care Campus

We are working in partnership with Baystate Health to create a unique medical campus for residents of East Longmeadow, Longmeadow, and surrounding communities. The collaborative project will provide efficient, coordinated, compassionate care to adults with medically complex conditions and ensure clinically appropriate transition and transfer of care.

Construction is underway for a new medical office building that will house Baystate Health specialty practices, primary care services, and related outpatient supports. The campus will also include a brand-new, state-of-the-art skilled nursing center with all private rooms for post-acute and long-term care. The existing East Longmeadow Skilled Nursing Center will be retrofitted to house specialized assisted living for those living with Alzheimer’s disease or related dementias.

Our specialized clinical programs and targeted capabilities — including pulmonary programs, dementia special care, continuous infusion treatment, and behavioral health — provide exceptional value to our partners.
